The Community Rehabilitation Service helps people to maximise their full potential for independence after being admitted to hospital. This service will support you if you have a physical disability, are feeling frail or if you are chronically ill or recovering from a traumatic injury.

Our focus is on helping you to regain or maintain the best possible level of function and independence so that you can return to activities that you could do previously. Your care will be tailored to your needs and will focus on goals that can be achieved in a limited time frame. The overall aim of the service is to improve your health and allow you to remain or return to your home and feel a part of your community.

Our approach is flexible and we will respond to your needs in a timely fashion. Your health will be the focus of our team of health professionals. By focusing on you, the service strives to reduce and prevent visits to the emergency department or admissions into the hospital. If you do need to enter the hospital, our approach to rehabilitation can help to reduce your length of stay.

Specialty services

  • Continence services – support from continence nurse advisors and continence physiotherapists
  • Cognitive, Dementia and Memory Service (CDAMS) – assessment and management of cognitive and memory change. CDAMS can be contacted directly on (03) 5722 1784.

Individual therapy

Therapy options for individuals include:

  • physiotherapy – for neurological, major orthopaedic, Parkinson’s, cardiopulmonary, continence, chronic illness and aged-related conditions
  • occupational therapy – functional rehabilitation for anyone with chronic and/or complex conditions, to improve independence within the community; this can include home modifications, equipment prescription, and neurological, upper limb or cognitive therapy
  • social work
  • dietetics – malnutrition treatment, nutrition support, weight management, diabetes management (together with a diabetes educator), practical cooking skills and education
  • gerontology assessment
  • specialist rehabilitation assessment
  • speech pathology – assessment and management for people who have communication and/or swallowing difficulties.

Therapy groups

Group therapy options include:

  • cardiac rehabilitation – for those who have had a heart attack, have stable heart disease, or have had angioplasty, stenting, or thoracic/cardiac surgery
  • pulmonary rehabilitation – for all long-term lung conditions and reconditioning before and after a lung transplant
  • Hearts for Life – for those who have experienced heart failure
  • Parkinson’s maintenance therapy – an exercise program for people with Parkinson’s disease
  • falls and balance program – to improve stability, strength and balance and develop falls prevention strategies
  • orthopaedic group – to help after major orthopaedic hip or knee surgery

Serious Mental Illness Laughter and Exercise Group (SMILE) – a program for people with chronic mental health illness that aims to promote exercise and to prevent the development of chronic conditions.

This service is available to all ages and can be accessed through:

  • Northeast Health Wangaratta hospital (inpatients)
  • Community Care Care
  • Residential Aged Care Facilities (if appropriate)
  • NHW is a registered provider for National Disability Insurance Scheme (NDIS), TAC and Work Cover.

Referrals: Anyone can make a referral to this service including all healthcare professionals, you or your carer. For self-referrals, a health summary from your GP should be included.

Waiting Times: All referrals to our services are assessed and prioritised according to your care needs.

Cost: We offer both Government-funded and client-funded services. Fees are kept to the minimum amount and will vary between programs. Information regarding relevant fees will be provided to you individually.
