If you have diabetes or are at risk of developing diabetes, our team of educators can work with you and your family/carer to identify appropriate goals and help you develop skills to self-manage your condition.
Diabetes educators can increase your understanding of risk factors and how to look after yourself by:
- Explaining diabetes to you
- Teaching you why it is important to monitor the amount of sugar in your blood
- Teaching you when to test your blood
- Providing information on medications for diabetes
- Teaching you how to manage your medications to maintain your sugar levels
- Offering information on how you can manage your sugar levels when you are sick
- Providing information on how healthy eating and exercise can help keep you well
- Helping you to avoid other health problems
- Explaining how you can avoid low and high sugar levels if you have type 1 diabetes
- Provide information and education on continuous glucose monitoring systems and insulin pump therapy, as well as provide support with the use of these devices.

- Community Care Centre
- Residential Aged Care Facilities (if appropriate)
Cost: We offer both Government-funded and client-funded services. Fees are kept to the minimum amount and will vary between programs. Information regarding relevant fees will be provided to you individually.
