If you have diabetes or are at risk of developing diabetes, our team of educators can work with you and your family/carer to identify appropriate goals and help you develop skills to self-manage your condition.

Diabetes Education

Diabetes educators can increase your understanding of risk factors and how to look after yourself by:

  • Explaining diabetes to you
  • Teaching you why it is important to monitor the amount of sugar in your blood
  • Teaching you when to test your blood
  • Providing information on medications for diabetes
  • Teaching you how to manage your medications to maintain your sugar levels
  • Offering information on how you can manage your sugar levels when you are sick
  • Providing information on how healthy eating and exercise can help keep you well
  • Helping you to avoid other health problems
  • Explaining how you can avoid low and high sugar levels if you have type 1 diabetes
  • Teaching you how to manage type 1 diabetes with the Dose Adjustment For Normal Eating (DAFNE) program.
  • Provide information and education on continuous glucose monitoring systems and insulin pump therapy, as well as provide support with the use of these devices.

Our services can be accessed through:

  • Northeast Health Wangaratta (inpatients)
  • Community Care Centre
  • Residential Aged Care Facilities (if appropriate)
  • NHW is a registered provider for TAC and WorkCover.

Referrals: Anyone can make a referral to this service including all healthcare professionals, you or your carer. For self-referrals, a health summary from your GP should be included.

Waiting Times: All referrals to this service are assessed and prioritised according to your care needs.

Cost: We offer both Government-funded and client-funded services. Fees are kept to the minimum amount and will vary between programs. Information regarding relevant fees will be provided to you individually.
