Join the NHW Heart Failure Collaborative as a lived experience partner

We are inviting expressions of interest from people living with heart failure who have had a Northeast Health Wangaratta (NHW) hospital admission in the last 3 years, or their carer or family member.

You will be an integral part of the project team, working with us to help plan and design the Heart Failure Collaborative at NHW, using your lived experience.

About the Heart Failure Collaborative:

Safer Care Victoria and the Institute for Healthcare Improvement are partnering with Northeast Health Wangaratta, and 29 other Victorian health services to embed evidence-based models of care that improve the management of and increase the time spent in the community for, those living with heart failure.

This state-wide initiative will run for 18 to 24 months and is part of the Reducing avoidable acute care admissions package.

Your new role:
As a lived experience partner, you will:

  • Bring the consumer or carer perspective into the NHW Heart Failure Collaborative. The team would value the consumer’s lived experience, to understand the physically, emotionally and socially experience during and after admission to hospital.
  • Foster connections with consumers and clinicians
  • Attend regular planning meetings (approx. 1-2 hour duration, approximately once per month). These meetings could be attended either in person or via an online platform such as Microsoft Teams or Zoom.
  • The consumer may have the opportunity to participate in education opportunities through Safer Care Victoria.

About you:
Ideally, the consumer will have lived experience as either a heart failure patient at NHW or as the carer of a heart failure patient at NHW.
