The Transition Care Program (TCP) can provide short-term support services following a stay in hospital. If you are aged 65 years or over (or with aged related disabilities) the program offers additional support so you can continue your recovery in a non-hospital environment such as your home or at Illoura Residential Aged Care facility while appropriate long-term care is arranged.
You will be allocated a case manager and receive an individualised care plan that may include a combination of services such as;
• case management
• domestic home care, such as cleaning services
• meal assistance
• help with bathing and showering
• organisation of appointments (including transport)
• social activities
• maintenance therapy as provided by allied health staff such as a physiotherapist
• continence aids
• equipment, as recommended by the physiotherapist or occupational therapist, such as a shower chair
• in–home respite.
Your case manager will review this plan with you regularly and your length of time on the program will vary depending on your needs and goals.
Eligibility: To be eligible for Transition Care Program you must;
- be assessed and approved by the Aged Care Assessment Service (ACAS) and the TCP team while you are in hospital
- have completed your hospital stay in a public or private hospital
- be medically stable and ready for discharge at the time you are assessed
- enter straight into TCP when discharged from hospital.
Referrals: Before the Transition Care Program team can assess your suitability for a package, you will need to be referred to the Aged Care Assessment Service (ACAS) by your doctor or other health professionals. Enquiries can be made directly to our TCP team on +61 (03) 5722 5390
Cost: The Australian Government determines the fees for the Transition Care Program. The amount contributed will be discussed with you when you meet with your case manager. Your access to the program will not be disadvantaged if you are experiencing financial difficulties or are unable to pay.
