At Northeast Health, we have a dedicated team of allied health, nursing, and medical professionals offering a range of community-based services to people of all ages.
We work with you as an individual to assess your health needs and help you achieve your desired goals. This may include attending a specialist clinic, participating in one of our specialised programs or group activities that support your personal therapy plan.
Waiting times to access our services can vary and the wait times provided for individual services are an estimate only. All referrals to our services are assessed and prioritized according to your care needs.
We offer both Government-funded and client-funded services. Fees are kept to a minimum and vary between programs. You will receive information regarding relevant fees.
We offer the following supports for patients and clients:
- An interpreter service is available to all clients and/or their representatives. Please let us know if you require an interpreter so that we can arrange one for your appointment.
- Aboriginal Liaison Officer
- Disability Liaison Officer
Information for patients and clients
The following guide outlines the steps we take when we receive your referral.
Step 1 - Referral sent
We receive a referral from your GP, Specialist or other service provider. They can send your referral electronically, by fax or post.
Step 2 - Assessment
We assess your referral to ensure that it has all of the information that we need.
Referrals that are incomplete, or do not have the required information to properly assess the referral, will not be accepted. If your referral is not accepted we will notify you and your referrer. Your referrer may need to send through a new referral with all of the information required.
Step 3 - Triage
If your referral is accepted it will be triaged. Triage is how we determine how soon you need to be seen by our service.
There are 2 triage categories for our clinic appointments:
- Urgent: you will get an appointment within 30 days
- Routine: we will contact you when an appointment is available
Appointments are available in person, via telehealth and in some cases by telephone.
Step 4 - Notification
We send a letter to you and your GP to let them know if your referral has been accepted.
Carparking
Client carparking is located off Clark Street as well as free 3-hour street parking along Clark Street.
Disability parking and access
A drop off zone and disability carpark are located near the front entrance.
There is a lift to access the first floor, however this is not large enough to fit a mobility scooter. If you will be using a mobility scooter to access the Community Care Centre, please call us on 5722 5555 so we can assist you with access.
Public transport
The nearest bus stop is at the front of the hospital on Green Street. From here, it is 500m to the Community Care Centre along Cusack Street. There is a slight downhill slope towards the Community Care Centre.
Wangaratta Railway Station is 1.2km from the Community Care Centre. There are some steep hills to navigate along this route and you will need to cross multiple roads.

If you miss 2 appointments in a row or 3 appointments in a treatment course without contacting us, you will be discharged from the clinic and a new referral will be required before another appointment is offered. This the Department of Health Policy.
As a client or consumer, you are involved in the decisions made about your care. You can help us by responding to letters and requests from us as quickly as possible. Please arrive on time and be respectful to staff and other clients.
Please tell us if:
- you change your contact details, nominated GP and next of kin or medical decision maker;
- you want to be contacted by phone, email or post;
- you need a carer, family member or support person;
- you need an interpreter or support from the Aboriginal or Disability Support Officers;
- there are changes to your clinical condition(s);
- you need transport and/or accommodation support;
- you can't attend your appointment;
- you don't need our services;
- you do not want us to talk to your GP, referring clinician or other health service providers about your care or treatment
Referring clinicians (internal and external to Northeast Health Wangaratta) are responsible for:
- considering options available to you as a patient in the community setting and informing you of current treatment options, so you can participate in decision-making about your future care and treatments
- ensuring referrals to health services for non-admitted specialist services are made with your agreement as a patient, recognising the content of any advanced care directive and including that you have agreed that your personal and health information can be shared with the health service
- complying with statewide or local referral criteria and providing complete and accurate information on the referral request to streamline the referral process and patient access
- providing their contact details and indicating their preferred method of communication for correspondence with us
- continuing to monitor you condition and notifying us if there is a change in your condition that could affect the urgency of treatment or the care required
How are referrals prioritised at NHW?
Referrals are categorised as high priority if the patient's condition has the potential to deteriorate quickly, with significant consequences for health and quality of life if not managed promptly.
Referrals are categorised as routine if the patient's condition is unlikely to deteriorate quickly enough to have significant consequences for the person's health and quality of life if the service is delayed beyond 30 days.

I have been referred, when will I be notified of my appointment?
Once your referral is accepted, you will be notified via your preferred method of communication (post or email).
Your referral will be reviewed by a clinician to determine how urgently care needs to be provided. Depending on the clinical urgency of your condition, we will then contact you to advise you of an appointment time or to say that you have been placed on a waitlist for an appointment.
How long will I have to wait to get an appointment?
Each service has different wait times. We try to see all urgent referrals within 30 days. Please see the Community Care Centre service directory for more information on wait times.
My condition has gotten worse since I was referred, how do I notify you?
Community Care Centre services are not emergency medical services. If you require urgent medical care, call 000 and ask for an ambulance, attend your nearest emergency department, or visit your GP.
If your referring doctor feels you need an early appointment, a new referral can be sent to us, and it will be reviewed. If it is deemed that you require an earlier appointment, one will be provided to you.
What should I do before my appointment?
- Complete any scans, X-rays, or tests required before your appointment
- Obtain any test results needed for your appointment
- Make a list of any medications or dietary supplements you take
- Write down any questions you have for your clinician
What should I bring to my appointment?
- Your appointment letter or email
- Your Medicare card
- A complete list of medications you are taking
- Relevant test or scan results
- Your GP's name, address and phone number
- Your advance care directive (if you have one)
Will I be sent an appointment reminder?
Yes - if you have a mobile phone, you will receive an SMS reminder of your appointment one (1) business day before. This will confirm your appointment date, time, and clinic name.
Can I reschedule my appointment?
Yes - to cancel or reschedule, please call (03) 5722 5555, and our administration team will assist you.
If you do not attend two (2) consecutive appointments, or three (3) over a treatment episode without notifying us, you will be discharged from our service and require a new referral to be seen by our clinics.
Can I change my appointment to Telehealth?
Telehealth is available at our health service to enable easier access to appointments for our patients, particularly those who have a long distance to travel. If you prefer to have your appointment by Telehealth, please phone us on (03) 5722 5555 to make arrangements.
We may need to see you in person for your first appointment and if your condition requires a physical examination by your clinician.
How do I get to my appointment?
You will need to make your own transport arrangements to and from your appointment.
There is ample parking near the Community Care Centre, and some disabled parking spaces are available. See our contact page for more information regarding parking.
Can I bring a support person to my appointment?
Yes. You are welcome to bring one carer/support person if you:
- require assistance with communication
- have difficulty understanding medical information and instruction from your clinician
- have behavioural or sensory needs
- have support needs that require an expert carer

Our experienced midwives run childbirth education and parenting classes. During these classes, we will provide you with lots of information about preparing for labour and birth and what to expect during early parenthood.

Our Community Rehabilitation Program at the Community Care Centre or in your home can support you following a new illness or injury or a decline in function.
Therapy can be provided by a number of health professionals and we work with people to deliver goal-focused and time-limited rehabilitation.
Our service is not registered with the NDIS and we do not provide ongoing therapy services.
