Community Interlink provides Home Care services via your local hospital or health service. Our mission is to help seniors live comfortably and age gracefully in their own home.
Community Interlink services include:
Home & Garden
Routine services or one-off jobs to keep your home clean and garden maintained. We can also arrange any home modifications necessary to meet your care needs.
Support to help you to have a shower or bath, get dressed, get in and out of bed, use the lavatory and perform other personal grooming tasks.
Behaviour management care, counselling, respite and sourcing of specialised amenities.
Help to prepare meals, organise delivery of pre-prepared food, go grocery shopping and provide feeding assistance.
Home Nursing and Allied Health services such as Occupation Therapy, Physiotherapy, Podiatry, Speech Pathology, Nutrition and Dietetics.
Sourcing and maintenance of hearing, visual, eating and mobility aides, as well as support apparatus including handrails, commode chairs and shower/bath chairs.
Assistance to get to your next major city, or simply to go shopping and get around town.
Community or home-based time to recharge for you and your carer.
Help with form-filling, letter writing, coordination of bill payments and more.
Support to stay connected to your favourite social groups and pastimes, or to find new ones.
Falls prevention support and 24-hour call button alarm set up.
Connect to relaxation activities, schedule well-earned massages or talk-it-out with one of our trusted counsellors.
Home Care Package purchase restrictions are determined and governed by The Commonwealth of Australia.

Case Management Fees with NO hidden extras
Unlike many providers, we don’t have additional fees and charges. For example, we won’t charge you for calling us, or for moving an appointment. Our fees are open, transparent and all-inclusive.
We don’t charge basic daily fees, start-up fees, staff travel fees or add-on service fees.
As a public health organisation, we’re non-profit. That’s why our fees are always kept as low as possible and we never compromise our high standard of service quality.
