The Central Hume Primary Care Partnership (CHPCP) is a partnership of health and human service organisations across the Alpine, Benalla, Mansfield and Wangaratta local government areas in North East Victoria. Our partner organisations include local government, health services, aged care, education and social services sectors.

The Program Coordinator: Prevention and Early Intervention is responsible for leading, coordinating, facilitating, and contributing to a broad range of prevention and early intervention strategies across the Central Hume catchment and more broadly the Ovens Murray region as well as facilitating working groups to progress specific Central Hume PCP and member generated initiatives
